The fuel sight gauge assembly includes the metal housing, the glass tube, (1) red fuel level ball and (2) cork washers.  The assembly is PMA'd for PA-11 and PA-18 aircraft.  (Ball and cork washers may be purchased separate also if you just need replacements. (In the attached photo, the elbows and placard are not included in the assembly but may be purchased separately).
